How to Enroll?
Getting started is simple! Choose the payment option that suits you best:
- A one-time annual fee of $395 covers your membership for a full benefit year.*
- Prefer monthly payments? Subscribe for just $35 per month ($420 per year).**
- Our treatment coordinators are happy to assist with enrollment.
- As soon as your membership is processed, you can begin taking advantage of the exclusive savings and benefits.
- You can cancel at any time, and you won’t be charged in the future.
*Terms and conditions apply. Each following year will be renewed at $395/year. A $50 re-enrollment fee may apply for patients with lapsed membership. *To enroll a family of two, a flat rate of $750 will cover both parties. For each additional family member a fee of $325/person will apply. **On-time, active, and current payments must be reflected in the patient’s chart at the time of services being rendered to determine eligibility for discounts. Failure to keep current enrollment status may result in the loss of discounts retroactively.
